Release and Waiver of Liability for Roots & Readings LLC Services

For, and in consideration of, the receipt of information and mediumship services from a medium provided by Roots & Readings LLC. I, being legally competent and fully authorized and empowered to do so, do hereby release, acquit, and forever discharge Roots & Readings LLC and all participating employees connected with this company, from any and all actions, course of action, claims, demands, injuries, damages, costs, loss of service, expenses and compensation, on account of any and all known and unknown personal injuries, mental anguish or agitation, and damage claims to person or property resulting from or arising out of or related to services and ministerial services provided by Roots & Readings LLC and its agents, representatives, and/or employees in any way affecting the undersigned parties. 

Confidentiality 

I acknowledge that the confidential information that I share with Roots & Readings LLC will be carefully guarded. Pursuant to Wisconsin Statutes 48.981 and 46.90, 55.043, Roots & Readings LLC may be required to disclose otherwise confidential information in certain situations, including: 

Incidents or suspicions of child abuse (physical or sexual) or neglect, or abuse or neglect to the elderly and mentally disabled individuals). 

Legal proceedings, including but not limited to filed court cases or served subpoenas, in which Roots & Readings LLC is deemed to not have the legal privilege of confidentiality. 

Waiver of Liability

I, having sought Roots & Readings LLC’s services, hereby acknowledge that I understand and agree to the following conditions and further releases from liability that Roots & Readings LLC and its employees, from any claim or litigation whatsoever arising from my participation in the above mentioned program. Further, I understand and agree that:

All services will be provided by Roots & Readings LLC employees and not licensed therapists, social workers, psychiatrists, psychologists, or medical professionals. 

All services provided in this ministry are spiritually based and are not necessarily provided in adherence with any local or national psychological or psychiatric association.

No representation has been made either expressly or implied, that the services, as conducted by Roots & Readings LLC, are accepted as customary psychological and or psychiatric therapy with definitional terms used by those professions. 

I have read and understand the contents of this waiver, I fully consent to its terms, and I request the services provided by Root & Readings LLC. 

By signing below, I further acknowledge and understand that the services and ministerial services provided by Roots & Readings LLC are primarily spiritual in nature and are not intended as a substitute for medical treatment, therapy, psychotherapy, legal services, or law enforcement protection, where appropriate. I fully understand that the employees are at all times restricted to consultation and do not involve diagnosing, prognosticating, prescribing medication, treating disease, or taking any other action which will constitute the practice of medicine in this state.

Choice of Law

On rare occasions, a conflict may develop between an employee and a client. Any such conflict will be first resolved by non-binding mediation. If mediation is not successful, the parties agree to arbitration in the State of Wisconsin. Arbitration may be completed telephonically if both parties can be seen and heard during the arbitration hearing. 

E-Signature

Each party agrees that this Agreement, and any other documents to be delivered in connection herewith, may be electronically signed, and that any electronic signatures appearing on this Agreement, or other such documents, are the same as handwritten signatures for the purposes of validity, enforceability, and admissibility.
